Pre-Participation Physical Examinations for Athletes
Find information here that discusses what you need to have for a physical examination as a new athlete to St. John Fisher. The NCAA requires all student athletes who are beginning their initial season of eligibility to undergo a comprehensive medical examination or evaluation administered or supervised by a physician (e.g. family physician, NP, PA, team physician). The examination or evaluation must be administered within six months prior to participation in any practice, competition or out-of-season conditioning activities. New athletes that had a physical examination that was completed before March 15th of the year you will enter college will need to have a new examination or evaluation clearing you for athletic participation.
We require that you have your medical provider complete the St. John Fisher University Sports Clearance form (PDF) as it provides all the necessary information we require. Make sure the Mandatory Response for Student Athlete/Sports Physicals section is completed by your provider. Please be advised that if you utilize a clearance form other than our sports clearance form, you must be sure that there is a statement on that form that states that you are cleared for sports participation with no restrictions. A physical examination form with no statement regarding clearance/restrictions Will Not Be accepted as a sports clearance.
